knock noise when shifting to reverse

Q: i have a 92 caravan 3.0l 3 speed automatic transmission that makes a loud knock when shifting to reverse. i also hear it when on the highway. i let off the gas and hear it and i step on the pedal and hear it. i had one of the drive axles replaced and the other one was inspected but nothing is wrong with it. any ideas what it could be .
