Rear Axle / Brakes swappable?

Q: recently purchased my truck in wv and moved it to texas.. in 3 days so im sure the drivetrain is sound so far .. was wanting to change the rear drums for a set of disk brakes.. if its possible what will i need to change brakes rotors axles.. is the performance worth the effort and will i need any special tools.. shadetree mechanic here so not too technical thanks .
